APPOINTMENTS
Appointments may be made Monday through Friday between 8:00 AM and 4:30 PM by calling our office 828-586-7994.

Referral from your Doctor
If you are interested in a Sleep Studies appointment, a referral from your family doctor is not required.

If you are interested in a Pulmonary appointment, a referral from another doctor is not required, but, your medical records must be received in our office before your appointment date.

First Time Patients
First, thank-you for choosing our practice, we know your time is valuable and will make every effort to make your appointment as productive as possible.

We hope the following information will be of assistance.

Please download and complete the Patient History Form with as much detail as possible, prior to your appointment. You should plan to arrive 15 – 30 minutes early to complete any additional paperwork that may be necessary.

Also, please bring the following items to your appointment:

• Insurance card(s)
• Referral forms (if required)
• List of all medications you are taking – please include all inhalers, nebulizer
• medications, and over-the-counter medications
• Pertinent medical records
• Recent chest x-rays (within last 6 months) – if these were performed at Harris Regional Hospital, please let us know and we will gladly have them ready for your appointment
• Any required copayments

If you do not have a recent x-ray, please contact Karen in our office at 828-586-7994 and we will help arrange this for you before your appointment.

Cancellations
If you find that you are unable to keep your appointment at the scheduled date and time, please notify us as soon as possible. We request at least a 24 hour notice for cancellations.

Delays
A final note, our practitioners will make every effort to be on time for scheduled appointments. Please know that, in addition to seeing patients in our offices, we provide critical care services to seriously ill patients in the hospital. Inpatient emergencies will occasionally cause us to run late, or to have to reschedule your appointment. We appreciate your patience and understanding if this happens.
